Building Check-ins

Build reusable wellness and reporting forms for your athletes.

A check-in is a wellness or reporting form you build once as a reusable template, then schedule to athletes or groups on the calendar. Athletes answer it from the mobile app, and their responses land back on your dashboard and results screen.

Your check-in library

Open Check-ins to see every template - name, question count, and whether it's Active or Inactive. Inactive templates stay in your library but can't be newly scheduled.

Building a template

Each template has a name, a description, and an Active toggle. Below that, add questions with a drag-and-drop list - see Question Types for what each widget captures. Save with Create template (or Save changes when editing).
